Soliciting Confidential Letters of Evaluation

Merit and Promotion Evaluation Request form using Qualtrics Surveys

For promotions, accelerated promotions, term reviews seeking indefinite status, career review advancement to full title VI or above scale, and career equity reviews.

Process for Soliciting Confidential Letters of Evaluation:

1. By October 28, 2025: Academic identifies three to five names of potential internal, external academic, and external clientele/partner evaluators. Academic provides the names and contact information to Academic Human Resources using the Qualtrics survey link: Coming Soon!

 

2. By October 28, 2025: Supervisor(s) recommends an additional three to five evaluators via a Qualtrics survey. Supervisors may seek input from County Directors, Program Teams, and Statewide Programs, as needed, on appropriate evaluators for the Academic. Supervisor provides the names and contact information to Academic Human Resources using the Qualtrics survey link:  Coming Soon!

 

3. By November 17, 2025: Associate Vice President and Vice Provost for Academic Personnel review names of evaluators submitted for each Academic and provide a confidential list to Academic Human Resources. The goal is to obtain a minimum of four letters of evaluation per Academic. Some evaluators recommended by the Academic and the Supervisor may not be contacted. The Associate Vice President and Vice Provost reserve the right to identify additional subject matter evaluators to help ensure a fair and objective evaluation.

4. By January 26, 2026: Evaluators must send their letters of evaluation to Academic Human Resources as described in the initial request by the Associate Vice President and Vice Provost for Academic Personnel. Academic Human Resources will upload all letters received for each Academic to their case files.
